General Patient Journey

Registration & Check-in

Patient arrives and checks in at the registration counter. Demographics are verified, payments processed when applicable, and the SystmOne journey begins.

Screening & Assessment by Diabetic Educator

Diabetic educators, nurses, and medical assistants record vital signs, anthropometry, and screen for NCD complications using standardized templates.

Doctor Consultation

Medical officer reviews history, investigations, and control status. Medications are adjusted, care plans updated, and referrals to FMS or allied health initiated when indicated.

Documentation & Prescription

Clinical notes are documented in SystmOne, e-prescriptions are generated via CCMS–PHIS integration, and the patient record is updated before the next step.

Follow-up Scheduling

The appointment counter schedules the next visit using the SystmOne rota based on clinical stability and required investigations. A TCA card is issued to the patient.

Pharmacy & Discharge

Medication is prepared and counselled by the pharmacist. The patient is discharged home with medication and a clear follow-up plan.
