These Terms of Use are the initial default for the CCMS Medical Documentation Hub. Site-specific clauses (e.g., MOH governance terms, contributor agreements, institutional disclaimers) will be added in a subsequent revision.

By accessing or using the CCMS Medical Documentation Hub (the "Site"), you agree to these Terms of Use ("Terms"). If you do not agree, do not use the Site.

1. Purpose of the Site

The Site is a documentation and reference resource for healthcare professionals, focused on the use of CCMS / SystmOne electronic medical record workflows and clinical operations within Malaysian public-sector clinics.

2. Eligible users

The Site is intended for:

  • Healthcare professionals (doctors, nurses, medical assistants, healthcare assistants, pharmacists, allied health staff)
  • Clinic administrators and operations staff
  • Authorised contributors and reviewers

The Site is not intended for use by patients seeking medical advice. Patients should refer to qualified healthcare providers.

3. Acceptable use

You agree to:

  • Use the Site only for lawful purposes consistent with your professional role
  • Not attempt to gain unauthorised access to any part of the Site, its infrastructure, or any related systems
  • Not interfere with the operation of the Site (e.g., scraping at abusive volumes, denial-of-service attempts, injection of malicious content)
  • Not submit feedback or contributions containing unlawful, defamatory, offensive, or sensitive personal data about identifiable patients
  • Not extract Site content for commercial redistribution without permission
  • Comply with all applicable Malaysian laws and your professional codes of conduct

4. Patient confidentiality

When submitting feedback, examples, or contributions, you must never include patient-identifying data (names, MyKad numbers, medical record numbers, photographs, dates of birth, or any combination that could re-identify an individual). The Site is not a clinical communication channel and is not designed for protected health information.

5. Intellectual property

5.1 Site content

Original content authored for the Site (text, diagrams, components) is the property of its respective authors and contributors, made available for reference within the healthcare community. Reproduction for personal study, internal training, or peer reference within Malaysian healthcare settings is permitted with attribution.

5.2 Third-party content

Where content references or quotes external sources (MOH guidelines, WHO publications, journal articles, vendor documentation), copyright remains with the original publisher. Such content is used under fair-dealing provisions for educational reference.

5.3 Trademarks and logos

The Ministry of Health Malaysia (Kementerian Kesihatan Malaysia, KKM) logo, system trademarks (SystmOne, etc.), and clinic-specific marks are the property of their respective owners. Their appearance on the Site does not imply endorsement.

6. Contributions

If you submit content (feedback, corrections, draft pages, comments), you grant the Site operator a non-exclusive, royalty-free, perpetual licence to use, modify, and display that content in connection with the Site. You confirm that the contribution is original to you or that you have the right to share it.

7. Disclaimers

Use of the Site is subject to the Medical Disclaimer, which forms an integral part of these Terms. In summary:

  • Content is provided "as is" without warranty of any kind
  • The Site is not a substitute for professional medical advice, institutional protocols, or current MOH guidelines
  • Reliance on Site content is at your own risk

8. Limitation of liability

To the fullest extent permitted by Malaysian law, the Site operator, authors, contributors, and any associated institutions shall not be liable for any direct, indirect, incidental, consequential, special, or punitive damages arising from use of or inability to use the Site, including but not limited to loss of data, loss of revenue, or clinical decisions made in reliance on Site content.

9. Privacy

Your use of the Site is also governed by the Privacy Policy, which describes how personal data submitted through the Site is collected and handled.

10. Modifications to the Site

We reserve the right to modify, suspend, or discontinue any part of the Site at any time without notice. We are not liable for any consequences of such changes.

11. Modifications to these Terms

These Terms may be updated from time to time. The "Last updated" date at the top reflects the most recent revision. Material changes will be highlighted on the Site. Continued use after changes constitutes acceptance.

12. Governing law

These Terms are governed by and construed in accordance with the laws of Malaysia. Any disputes arising from use of the Site shall be subject to the exclusive jurisdiction of the courts of Malaysia.

13. Severability

If any provision of these Terms is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
