Medical Disclaimer
Last updated: 2 May 2026
The information on this Site is provided for educational and informational purposes only and is not a substitute for professional medical judgement, institutional clinical protocols, or up-to-date Ministry of Health (MOH Malaysia) guidelines.
1. No medical advice
Content on the CCMS Medical Documentation Hub (the "Site") describes generalised workflows, system documentation, and reference protocols. It is not intended to:
- Diagnose, treat, cure, or prevent any disease or medical condition
- Provide individualised medical advice to any patient
- Replace the clinical judgement of a registered medical practitioner
- Override official institutional Standard Operating Procedures (SOPs), MOH circulars, or current Clinical Practice Guidelines (CPGs)
2. No doctor-patient relationship
Use of this Site does not create a doctor-patient relationship between you and any author, contributor, or operator of the Site. Content is contributed by clinical staff for peer reference and may reflect the practices of specific clinics. It does not constitute personalised medical advice.
3. Always defer to current authoritative sources
Where Site content references clinical protocols, drug dosages, diagnostic criteria, or coding standards, you must verify the information against:
- The most current MOH Clinical Practice Guidelines (CPGs)
- Active MOH circulars and directives
- Your own institution's current Standard Operating Procedures
- The current product information for any pharmaceutical or device
Medical knowledge and guidelines change. Site content is reviewed periodically (review status is shown per page where applicable), but readers must independently confirm currency before clinical application.
4. Use at your own risk
Reliance on any information provided through the Site is solely at your own risk. The Site operator, authors, contributors, and any associated institutions:
- Make no warranty regarding the accuracy, completeness, currency, or fitness-for-purpose of any content
- Are not liable for any direct, indirect, incidental, consequential, or punitive damages arising from use of, or inability to use, the Site
- Are not responsible for errors, omissions, or outdated information
5. Emergencies
This Site is not for emergency use. If you or someone in your care is experiencing a medical emergency, contact emergency services (call 999 in Malaysia) or proceed immediately to the nearest emergency department.
6. External links
The Site may contain links to external resources (MOH, WHO, journal articles, etc.) for reference. Inclusion of an external link does not constitute endorsement, and we are not responsible for the content, accuracy, or practices of external sites.
7. Authoritative content vs. peer-shared content
Not all content on the Site has the same authority:
- Clinical protocols and SOPs marked with a clinical reviewer (
Reviewed by ...) reflect the practice of the contributing clinic at the time of review. They remain peer-shared content, not nationally authoritative. - CCMS system documentation describes how a particular electronic medical record system is used; it is operational, not clinical, in nature.
- FAQ and tutorial content is intended as practical guidance for users of the system, not clinical instruction.
When in doubt, consult your institution's clinical leadership or the relevant MOH guideline.
8. Reporting concerns
If you identify a clinical error, outdated content, or safety concern in any page on this Site, please report it via the feedback widget on the page in question or by contacting us at ilmpressnet@gmail.com. We take such reports seriously and will review promptly.
9. Changes to this Disclaimer
This Disclaimer may be updated from time to time. The "Last updated" date at the top reflects the most recent revision. Continued use of the Site after changes constitutes acceptance of the revised Disclaimer.
