How to Access Deleted Items and Reinstate Entries

Quick Answer

Navigate to the patient's Administrative Tree → Deleted Items to view entries marked in error. Right-click and select Reinstate to restore a mistakenly deleted entry.

What Problem Does This Solve?

You need to review entries that were marked in error, audit past corrections, or you accidentally marked an entry in error and need to restore it to active clinical view.

Before You Start

Prerequisites
  • View Deleted Items permission on your role
  • The entry was marked in error from your organization
  • Authorization to reinstate (if required by your clinic policy)
  • Note: Medications and consent entries typically cannot be reinstated

Access Deleted Items

Open the Patient Record

Search and verify you are in the correct record

  1. Use Quick Patient Search (Ctrl + F) to find the patient.
  2. Open the patient record.
  3. Verify the patient name and IC number match.

Navigate to the Deleted Items Node

Locate the Administrative Tree in the patient record

  1. In the patient record, locate the Clinical Tree panel on the left.
  2. Scroll down or switch to the Administrative section.
  3. Find and expand Deleted Items.
Can't See It?

If Deleted Items is not visible, you likely lack the required permission. Contact your clinic administrator.

Review Deleted Entries

Examine details before deciding to reinstate

Each entry in Deleted Items shows:

  • Date and time the entry was marked in error
  • User who performed the action
  • Entry type (consultation, observation, code, etc.)
  • Reason provided when marking in error
  • Original content (visible when you click the entry)

Each entry in Deleted Items shows: Date/Time, related User who performed the action, entry type, reason & the original content.

Click any entry to view its full details. Only proceed with reinstatement after confirming the entry was marked in error mistakenly.

Reinstate a Mistakenly Deleted Entry

Restore the entry to active clinical view

  1. Right-click the entry you want to restore.
  2. Select Reinstate from the context menu.
  3. Confirm the action when prompted.
  4. The entry returns to active status in its original clinical node.
Limitations
  • Medications and consent entries typically cannot be reinstated
  • Cross-organization entries cannot be reinstated
  • Same-session deletions may block reinstatement by design

Verify the Reinstatement

Confirm the entry is restored and visible

  1. Navigate back to the relevant clinical node where the entry originally appeared.
  2. Confirm the entry is now active and visible.
  3. Return to Deleted Items and confirm the entry is no longer listed there.
  4. Check that the audit trail records the reinstatement action.
Audit Record

Both the original mark-in-error and the reinstatement are permanently logged in the audit trail.

Troubleshooting

Cause: Your role lacks View Deleted Items permission.

Solution:

  1. Contact your clinic administrator to verify your permissions.
  2. Request the View Deleted Items right if your role requires it.
  3. If access is denied due to policy, ask an authorized colleague to review on your behalf.

Cause: The entry type cannot be reinstated, or it was deleted in the same session.

Solution:

  1. Verify the entry is not a medication or consent record.
  2. If it is a same-session deletion, wait a few minutes and try again.
  3. If still unavailable, the entry may be cross-organization — contact ICTSO.

Cause: You are concerned about clinical or audit implications of restoring the entry.

Solution:

  1. Consult your supervisor before reinstating.
  2. Document the reason for reinstatement in the patient journal.
  3. Ensure the reinstatement aligns with clinical governance policies.
