When a prescription error is identified (wrong drug, dose, patient, or duplicate), follow this protocol.

Pharmacist flags discrepancy

During PHIS verification, the pharmacist identifies the error

Medication Error
  • Medication dosage error
  • Wrong patient prescription error
  • Wrong medication duration
  • Enquiry regarding drug-interaction

Pharmacist contacts prescriber

Directly or via SystmOne medication query

  • Call directly via phone
  • Message via CCMS Messaging Feature
  • Send Medication Query ― Mandatory

Doctor amends in SystmOne

Corrected prescription re-syncs to PHIS

Amendments
  • Choose the wrong medication --> Stopped Medication 🔴
  • Represcribe with correct dose/duration
  • Save patient profile

New medication detail sent to PHIS

New medication entry has been created

  • Medication that has been stopped via CCMS will be stopped as well in PHIS (automated sync)
  • New represcribed medication entry is now available in PHIS

Both systems preserve history

Full traceability is maintained

Critical Limitation

Corrections made directly in PHIS by a pharmacist will create a discrepancy between systems. Doctors must always amend prescriptions in SystmOne to maintain a single source of truth.
