Pharmacist flags discrepancy
During PHIS verification, the pharmacist identifies the error
- Medication dosage error
- Wrong patient prescription error
- Wrong medication duration
- Enquiry regarding drug-interaction
Pharmacist contacts prescriber
Directly or via SystmOne medication query
- Call directly via phone
- Message via CCMS Messaging Feature
- Send Medication Query ― Mandatory
Doctor amends in SystmOne
Corrected prescription re-syncs to PHIS
- Choose the wrong medication --> Stopped Medication 🔴
- Represcribe with correct dose/duration
- Save patient profile
New medication detail sent to PHIS
New medication entry has been created
- Medication that has been stopped via CCMS will be stopped as well in PHIS (automated sync)
- New represcribed medication entry is now available in PHIS
Both systems preserve history
Full traceability is maintained

Corrections made directly in PHIS by a pharmacist will create a discrepancy between systems. Doctors must always amend prescriptions in SystmOne to maintain a single source of truth.
