Documentation, Reporting & BCP Drill
All BCP activities must be properly documented and periodically tested to ensure clinic-wide readiness and compliance with KKM and ISO standards.
Documentation & Reporting SOP
Completed BCP Report (Lampiran A)
Prescriptions, clerking sheets, consent forms, logbooks, Google Sheet/Excel trackers
Records must be stored for at least 2 years for audit purposes.
BCP Manager is responsible for submitting reports and filing all records securely.
BCP Drill Requirement
Minimum Frequency
Once per year or as directed by KKM
All units must participate in at least one BCP drill annually.
Alternative to Planned Drill
Real downtime events may substitute
A real downtime event may substitute for a drill if fully documented using Lampiran A.
Reporting
Documented and reviewed by clinic management
Drill outcomes must be documented using Lampiran A and reviewed by clinic management.
The official BCP Report Template (Lampiran A) is referenced throughout this documentation. It is obtained from the Muar ICT Helpdesk App and must be completed for every BCP activation or drill.
